چکیده
The heavy light chain (HLC) immunoassay quantifies light chain types of each immunoglobulin (Ig), enabling the calculation of ratios of HLC pairs (Katzmann & Rajkumar, 2013). This assay measures the light chain types of each Ig (G/A/K), generating ratios of monoclonal Ig/background polyclonal Ig concentrations as paired ratios. It has shown clinical utility in multiple myeloma (MM), particularly for recognition of small M-proteins that migrate in the beta or gamma fraction among polyclonal immunoglobulins, for monitoring of minimal residual disease and as a predictor of progression of monoclonal gammopathy of undetermined significance to MM (Bradwell et al, 2013; Katzmann et al, 2013; Ludwig

Serum Free Light-chain Assay for the Diagnosis, Management, and Prognosis of Multiple Myeloma
In recent decades, new serum biomarkers have been developed for routine laboratory practice, such as assaying serum free light chains and more recently, assaying immunoglobulin heavy and light chain isotypes (Hevylite). In this work, we highlight the interest of new biomarkers (Hevylite Test) in the management of monoclonal gammopathies because of the technical advantages it confers and the se...
متن کاملAbnormal heavy/light chain ratio after treatment is associated with shorter survival in patients with IgA myeloma
Immunoglobulin (Ig) heavy/light chain (HLC) assays enable the separate quantification of the different light chain types of each Ig class. We retrospectively analyzed the correlation of heavy/light chain ratio (HLCR) with clinical status and its impact on outcome in 120 patients with multiple myeloma (MM). Abnormal HLCR was seen more frequently in patients with poorer myeloma response, and it a...
متن کاملCombined use of free light chain and heavy/light chain ratios allow diagnosis and monitoring of patients with monoclonal gammopathies: Experience of a single institute, with three exemplar case reports
Monoclonal gammopathies are characterized by serum monoclonal component (MC) plus an intact immunoglobulin and a free light chain (FLC), or a combination of both. The measurement of FLC with Freelite® is the standard practice recommended by International Myeloma Working Group guidelines. Recently, Hevylite® heavy/light chains (HLC) assays were introduced to specifically target junctional epitop...
متن کاملA Tentative Staging of Multiple Myeloma by Utilizing Respective Coefficients of Prognostic Factors
Introduction: Multiple myeloma is a heterogeneous disease with different survival times among patients. Accurate prediction of prognosis in multiple myeloma is essential, as patients with a shorter survival time may require early bone marrow transplantation (BMT) and more advanced chemotherapy as a part of their first-line treatment. In the present study, a parameter, depicted by ga...
